The American Academy of Pediatrics
The American Academy of Pediatrics and the American College of Emergency Physicians have developed a two-page Emergency Information Form for Children with Special Needs. The form is designed to be filled out by the child’s doctor as a precaution before a crisis arises. When presented during an emergency, it can give the emergency professionals a quick overview of the child’s needs. The form is available online from the American Academy of Pediatrics at or from the American College of Emergency Physicians.
